Fieldhome has become the region’s leader in sub-acute rehabilitation and is now the facility of choice for short-term rehabilitation. We provide the most aggressive and comprehensive post-surgical, sub-acute rehab program in our discrete 50 bed rehabilitation unit. This unit is focused on four sub-specialties: orthopedic, neurological, cardio-pulmonary and post-surgical rehabilitation.

Our specially trained rehabilitation professionals will design a customized 7-day-a-week therapy program that meets your individual needs and will work with you so that your stay at Fieldhome is as beneficial and brief as possible.

We now also provide the extra oversight of a Nurse Practitioner as the Rehab program’s Sub-Acute Clinical Director. This is a newly created position that will result in continuous collaborative communication with our patient’s physicians, decreasing the chance of re-hospitalization and minimizing the patient’s length of stay. Fieldhome is the only sub-acute rehabilitation facility in the region to have such a strong medical presence in its rehab program.

We want your stay with us to be as enjoyable and comfortable as possible. When you arrive at Fieldhome for your rehabilitative care you are immediately assigned a Personal Concierge, who will help you settle into your room, contact your physical therapist for same day evaluation and treatment and keep in contact with you throughout your stay to address any needs you may have.
Our beautifully decorated Center offers many amenities for you to enjoy, including the use of portable DVD players and current movies, laptops with wireless internet service, phone and cable television, recreational programs, educational seminars, and your choice of eating your meals in your room, in the Rehab Center’s dining room or in a private dining room with your family.
