Hello and Welcome to Fieldhome.

Thank you for visiting our website and allowing me to preview and “boast” a little about our exceptional community of healthcare services.

At Fieldhome, we provide the most aggressive and comprehensive post-surgical rehab programs, outside the sub-acute hospital setting, in our discrete 50 bed rehabilitation unit. This unit is focused on four sub-specialties: orthopedic, neurological, cardio-pulmonary and post-surgical rehabilitation. This focus on clinical specialization is unmatched elsewhere.

We now also offer the extra oversight of a Nurse Practitioner as the Rehab program’s Sub-Acute Clinical Director. This is a newly created position that will result in continuous collaborative communication with our patient’s physicians, decreasing the chance of re-hospitalization and minimizing the patient’s length of stay. Fieldhome is the only sub-acute rehabilitation facility in the region to have such a strong medical presence in its rehab program.

Our traditional 150 bed long-term skilled nursing facility, Fieldhome – Holy Comforter, is mission driven and committed to the ideals of person centered care — a commitment to the value of individualized attention and consistent staff assignments. Our long established reputation of nursing excellence is one that is built on our 130+ years of experience.

The Seabury at Fieldhome is a luxurious residence offering exceptional assisted living accommodations for those seeking an independent and care-free lifestyle. The Seabury also provides a specially designed and secured memory support environment geared to the needs of those with mild to moderate dementia.

Lastly, we are home to the Early Learning Center at Fieldhome, an accredited child learning and care program, accommodating children from ages 6 weeks to 5 years old. This exceptional center supports our intergenerational program which brings together the youngest and oldest generations, in ways that are both educational and nurturing.

Each of these outstanding programs resides on our beautiful 100 acre wooded campus. A setting that offers beautiful landscaping, privacy and security. To this, we add our friendly and professional staff who are consistently acknowledged for their warm and caring ways.
