REHABILITATION SERVICES DIRECTOR

Fieldhome, a 200 bed skilled nursing facility located in Northern Westchester County on a 100+ acre campus is seeking a dynamic professional and dedicated team player for our Director of Rehabilitation Services.  This position will manage the effective and efficient delivery of rehabilitation services;  provide administrative supervision for the clinical and support staff & conducts the Rehab QA Program.  The ideal candidate will be possess knowledge of NYS DOH Regulations and Reimbursements.  Strong communication skills necessary.  Current registration and licensure as a therapist required.

PHYSICAL THERAPIST

PT needed for skilled nursing/rehab facility working with adult and geriatric population.  Duties include evals, implementation of daily treatment, daily and bi-weekly documentation.  Must possess strong analytical and communication skills.

NYC License & Registration req’d; BS or Masters degree as appropriate.  Sub-acute care and/or geriatric experience a plus; New grad also OK.  F/T Flexible schedule available.

CERTIFIED OCCUPATIONAL THERAPY ASSISTANT

Provides OT services to residents by assisting in the assessment, treatment, program planning and implementation, related documentation and communication.  Requires and AS Degree as an OT Assistant and national certification for OTA analytical skills, ability to communicate effectively orally and in writing, and empathetic in difficult interpersonal situations.  Current registration and current state licensure as an OTA are necessary.
