About Us
FIELDHOME is a vibrant center for health care and community services located on the border of Cortlandt Manor and Yorktown. Our beautiful 100+ acre campus serves the needs of our community from birth through the senior years with exceptional programs including: Child Care, Short-term and Long-term Rehabilitation/Skilled Nursing, Assisted Living, Memory Support, Respite care, Long-Term Care, and Hospice.

  • 202-bed Skilled Nursing Facility including a 152-bed Long-Term Care Unit and newly renovated 50-bed Rehabilitation Unit
  • Over 420 new Admissions in 2006, providing over 68,000 patient treatment days
  • New 1,500 sq. ft. Rehabilitation Treatment Center
  • ONLY facility with full-time Physiatrist to oversee Rehab Program

 

The Seabury at FIELDHOME

  • Licensed Assisted Living
  • Newly Constructed in 2003 encompassing: 40-bed Assisted Living Residence and 40-bed Memory Support (Alzheimer’s Care) Residence
  • Provides upscale accommodations and high quality care overseen by both a Nurse Practitioner Administrator and Physician Medical Director
  • Affordable housing - 10 Assisted Living Unit Beds dedicated to SSI/Medicaid recipients

 

The Early Learning Center at FIELDHOME

  • Registered and Licensed Program for 30 children ages 6-weeks to 5 years
  • Operates 12-hours/day year-round
  • Partners with FIELDHOME seniors in unique on-site Intergenerational Program
  • Program Director with Masters in Early Education, Montessori Certification and Teaching Background
